Follow the instructions to request a partial surrender or full surrender from your annuity contract.
Remember:
- All required signatures must be included.
- Make sure you鈥檝e clearly selected whether your request is for a gross or net amount.
- Choose only the applicable withholding option. Selecting multiple boxes may cause processing issues.
- Complete the community property section only if it applies to your account. If you鈥檙e not sure, review your account details.
- Complete form
Provide information according to the instructions on the form. - Submit form
Submit your documents according to the instructions on the form. - Allow 5-10 days to process
黑料不打烊 will process the request within 5-10 business days of receipt if in good order and confirmation of the payment schedule.
Please note: The processing time includes time to mail payment information to the policy owner. - Disburse
Payment will be mailed to the owner's address on record unless approved as a direct deposit payment.
