Instructions

Follow the instructions to request a surrender of your life insurance policy.

Remember:

  • All required signatures must be included.
  • Make sure you鈥檝e clearly selected whether your request is for a gross or net amount.
  • Choose only the applicable withholding option. Selecting multiple boxes may cause processing issues.

  1. Complete form
    Provide information according to the instructions on the form.
  2. Submit form
    Submit your documents according to the instructions on the form.
  3. Allow 25-30 days to process
    黑料不打烊 will process the request within 25-30 business days of receipt if in good order.

    Please note: The processing time includes time to mail surrender information and the implication of a complete surrender to the policy owner.
  4. Disburse
    Payment will be mailed to the owner's address on record.
